Do you participate in the Diversionary Work Program (DWP) or Work Benefit (WB) Program?

If you answered 'Yes' to any of the above questions, you may be able to get food support. Contact your county human services office (http://edocs.dhs.state.mn.us/lfserver/Legacy/DM-0005-ENG) or call (888) 711-1151.

If you answered 'Yes' to these questions, you may be able to get food support.

You will be required to complete the Combined Application Form (CAF) (www.edocs.dhs.state.mn.us/lfserver/Legacy/DHS-5223-ENG) and submit it to your county human services agency (http://edocs.dhs.state.mn.us/lfserver/Legacy/DM-0005-ENG).
